Fill out an Adoption Application

Baby came to us from the shelter in January. At first, was a timid, shy girl, but longed for love and affection; clearly something she had not had much of.
Once she warms up to you, she is a very social girl, with both humans and other dogs. She takes a few minutes with strangers, she wants to run up to them, but is a little gun-shy. 
Baby has a super sweet personality and is very submissive. She wants all your love and deserves the world. 
Baby is great with dogs and even cats. She will chase a cat occasionally but only wants to play; for the most part she totally ignores them. 
Baby is crate trained and absolutely loves kids. She is still working on housetraining, and we don't think she ever lived indoors before. She has made improvements while in foster care. She is doing better on leash training, but again this is all new to her. She doesn't pull but things still scare her so she might get afraid sometimes and try to run so must be careful and have control of her. (No retractable leashes!) 
Baby would be fine living in any sort of home environment, apartment, home with fenced or not as long as she is taken out and gets exercise and walks. She does prefer a companion animal to play with. 
She would prefer to go to a home with another easy-going dog to play with and loves the other dog in her foster home. It may help her come out of her shell, as the other dog gives her confidence. She likes to hang out in the yard on a tie out with her family now but when she came to PLP she was scared to be outside. 
While she is crate trained, she loves to sleep in bed next to a human. She is a total love bug and craves love and attention. 
Baby is about 3-4 years old and only weighs 20 lbs. 
She is spayed, chipped, utd on vaccines, dewormed, hw negative and received a 6 month heartworm preventative injection. She is current on flea meds and comes with a leash and collar. 
Her adoption donation is only $200.00 and she comes with all vet records. 
Approved application, home visit and reference check required.
