Fill out an Adoption Application

Hi, my name is Maddox!  My foster mom calls me Maddy for short, except when I’m not listening to her! I’m a ten-month-old, very smart, handsome, sweet, loveable boy. I have mesmerizing blue eyes, reddish brown hair, a sleek body and a curly tail that works like a slinky! I play with my tail all the time. I chase it in circles and when I catch it, I pull it out straight, then let go and it curls right back up. Pretty neat and lots of fun!
 I’m house trained, neutered, and crate trained.  I love going for walks and golf cart rides.  I enjoy meeting people and other dogs, and running around the back yard playing “catch me if you can.”  Off leash I do not run away, I stay in the yard. I bark at people walking their dogs but when I’m introduced to them, I stop and give kisses and play with their dog. I just love everybody! 

I live in an active foster home with seven kittens, three cats, and two dogs.  I just love them all! Especially the kittens-I want to play but my foster mom says they are too small for me. I love watching them play and sometimes I stick my nose in hoping to play too but they hit and scratch me and I run away. My favorite kitten lets me put my head next to her and give her kisses. Sometime I kiss her so much our foster mom has to dry her off! My foster mom says  I’m a sweetheart. I would make a wonderful addition to an active family who has other pets. Since I am a young guy I will need a lot of exercise! Whoever adopts me, I will just melt your heart! 
 

I am up-to-date on all vaccines, heartworm negative, micro chipped, and on preventatives. I was given a six month heartworm preventative injection called Pro-Heart so there are no monthly pills to keep up with. I come with all vet records, a new leash and collar, and a small bag of food to mix with whatever my new family wants to feed me as well as any personal items. I am located in the Myrtle Beach, South Carolina area. Approved application, home visit, and an adoption donation of $200 required.
